I don't know if this is just my autocad but I have had the same problem in both Autocad 2015 and 2016. Fairly often when I go to place a refnote or planting symbol they generate at the bottom of all the displayed geometry. so they are under all the geometry and xrefs in the file by default. Is there a way of preventing this?

If you immediately save the file, and close and reopen AutoCAD, is it still beneath the others?
If so, then please send it in, as we may be able to determine something.
Otherwise, we have only seen this happen occasionally, with it appearing to be an AutoCAD bug that we can't quite isolate.
